Over the years we have determined that by instituting a series of simple policies it has made it easier for both our patients and staff.  In some instances these have come about because of legal restrictions or insurance company regulations.  However, many of them reflect what we have determined to be good business practices that ensure that we will be here to provide quality services to our patients for many years to come.

 

Appointment Cancellations
Any appointment cancelled with less than 24 hours (or one business day's) notice is subject to a cancellation fee of $25.
 

No-Shows
Failure to show up for an appointment will result in a $25 charge on your account.
 

Medical Records Request
We are more than happy to provide you with copies of your medical records.  This request is subject to a fee of not less than $75.  Records take 10-14 days to prepare and the process cannot begin without a signed Medical Records Request form which is available from the Forms page on this website.
 

CoPayments
Copays are due at the time service is rendered.  If you are unable to pay at that time your appointment will be rescheduled.
