I work from 9-5. 
Is it possible to speak with someone after hours ?
One of our Physicians is on call at all times.  However, calls after business hours are for emergencies only.  Calls that are not considered emergencies include:  billing questions, prescription refills, scheduling issues, paperwork that you need, and problems that have been present for more than a week but not getting worse.

 
 
 
 

What is your appointment cancellation policy ?
It is important that you notify our office as soon as possible when you know you are going to miss a scheduled appointment so that we can make this slot available to someone else.  In the event that you notify us less than 24 hours in advance we charge a fee of $25.

 
 
 
 

At what age should my child have their first eye exam ?
We see patients of all ages in our practice.  Children should have their first exam prior to their 2nd birthday.

 
 
 
 

I need a written excuse for work or school.
This issue will be handled by the doctor at the time of your appointment as needed.

 
 
 
 

How do I get a prescription refilled ?
You may call our office or use the Rx Refill form on this website (see the button to the left).  When calling please make sure you have your pharmacy information available.  If you end up using our voice mail please speak clearly and slowly making sure you specify your name, a phone number where we can reach you, the medication you need refilled, pharmacy name, and pharmacy phone number.  Please note that prescriptions are not automatically refilled.  It is up to the doctor to decide whether or not to refill the prescription.  Our doctors are very busy and are not always available to immediately respond to these requests.

 
 
 
 

I need to get a copy of my medical records.
In order for us to provide you with a copy of your medical records you must sign a Release of Records form.  This can be downloaded from the Forms page on this website or from our office.  It is important to note that once you have executed this form it can take 10-14 days for us to copy these records for you.  In addition, because of the labor involved we must charge a fee for this service.  Please be sure to determine the specific cost for your record at the time you request this copy.

 
 
 
 

Can you bill me for my copay ?
Unfortunately we are required to collect your copay at the time services are rendered.  If you cannot pay your copay at the time of your visit it will be necessary for us to reschedule your appointment. It is illegal for us to forgive a copay - please do not ask us to do this. 

 
 
 
 

My doctor says I need surgery - how do I schedule it ?
The office will provide you with a letter of introduction to our  Ambulatory Surgical Center.  The Surgery Scheduler will discuss the process with you in order to make sure your scheduling needs are met.  PLEASE ALLOW 7-10 working days for her to contact you.  At that time she will arrange to get you all the paperwork you need and may ask that you see your Primary Care Physician in order to get a medical clearance prior to your surgery.
